Start Check-in: Begin a new check-in for yourself or an employee you supervise.

• Employee: Complete the self-evaluation form below. Click Save at anytime, to save your work and complete at a later time. To return to a previously saved check-ins, click In Process. Once you are ready to submit to your supervisor, click Submit.
• Supervisor: To send the self-evaluation to an employee, enter the name of the employee and select SUBMIT. An email notification will be sent to the employee requesting completion of the self-evaluation.
